You will find information below and in the sidebar regarding enrolling your child(ren) in Crookston Public Schools. Please complete all the forms and return them, along with the required information, to the school office. A parent or legal guardian must complete and sign each registration form. In order for the enrollment process to begin, we are requesting you provide the following information.

New Student Registration

If you are new to Crookston Public Schools you will need complete the following forms and provide a copy of your student(s) immunization records and birth certificate.

• MDE Ethnic and Racial Demographic Designation Form

• MDE Digital Equity Survey

• Application for Educational Benefits

• School Bus Request Form

  • Religious Education Bus Form (7th & 8th)

  • School Bus Change Form

• Crookston Migrant Survey

• Open Enrollment Form ( only for those that live outside the district)

To complete your registration please turn in all forms to the appropriate school building associated with your students grade.  

 

Returning Student Registration

Crookston Public Schools is utilizing online enrollment through Skyward our student software system.  You may register your student(s) by logging in to your Skyward Parent Portal.  If you are unsure what your login information is please contact your a school in which your student is attending.  Please keep in mind that we do have a paper option available for registration for those who may not have Internet access.  These additional forms are not a part of the online process and will need to be completed and returned.
