To complete your registration please turn in all forms to the appropriate school building associated with your students grade. You will find information below and in the sidebar regarding enrolling your child(ren) in Crookston Public Schools. Returning Student Registration Crookston Public Schools is utilizing online enrollment through Skyward our student software system. You may register your student(s) by logging in to your Skyward Parent Portal. If you are unsure what your login information is please contact your a school in which your student is attending. Please keep in mind that we do have a paper option available for registration for those who may not have Internet access. These additional forms are not a part of the online process and will need to be completed and returned. Offerings include: Utilizing two separate classrooms, two certified teachers serve up to eight students in a secured setting and up to eight students in a non-secure setting, coming from districts throughout northwest Minnesota Small group and independent study approaches are utilized Curricula is designed around students' educational needs Basic literacy skills remediation is emphasized Students have access to Minnesota Comprehensive Assessment II (MCA II) test preparation and administration Special Education services are available to students with special needs Transcripts of class hours earned by students are sent to students' home schools

Our school's Parent Teacher Organization (PTO) exists to enhance the school experience for Washington students. This group of volunteers sole purpose is to establish and nurture a close relationship between the parents, teachers, and students. Washington Parent Advisory Council The shift in grades at Washington School also brings about a change in the title and purpose to the parent group associated with Early Childhood Family Education and School Readiness. The Parent Advisory Council is seeking a diverse group of parents to assist in program planning and decision-making at our school. Involved parents will be supported and empowered through advocacy skills as family engagement is strengthened. Community professionals will also be welcome to provide input and guidance. Meetings will be held at Washington School from 5:00 to 6:00 p.m. Please notify the office in advance if you plan to request care for your child during the meeting by calling 281-2762. Below is her tentative schedule. Hours and location can vary. MONDAY/TUESDAY Highland 7:45am – 3:35pm WEDNESDAY/THURSDAY Crookston High School 7:45am – 3:35pm FRIDAY Washington 7:45am – 11:00am Highland 11:00am – 3:35pm MEDICATIONS Medications should be administered at home under the supervision of the parent/guardian. However, there are cases when students will require the administration of medication during the school day. Medication Guidelines Authorization to Administer Medications (To be completed by the Medical Provider and Guardian ) Self-Administration of Medication (To be completed by the Medical Provider and Guardian ) IMMUNIZATION REQUIREMENTS Minnesota law requires written proof of certain vaccinations for children in child care, early childhood programs, and school. However, if a child has a medical reason or if his/her parents are conscientiously opposed to any or all of the vaccinations, a legal exemption is available. MN Immunization Law Student Immunization/Conscientious Exemption Form STUDENT HEALTH FORM (To be completed by a Guardian ) IS MY CHILD WELL ENOUGH TO GO TO SCHOOL? These guidelines are intended to help you decide whether your child is healthy enough to go to school. Call your school health office if you have any questions about your child's illness. Contact you student's school every day they are absent. Your phone or email message should include: child's name, teachers name or grade, reason for absence, and please include their symptoms (fever, sore throat, ...). Your child must stay home if they have: Fever of 100 degrees or higher Vomited Diarrhea Rash that is not yet diagnosed Your child may return to school: 24 hours after last episode of vomiting or diarrhea 24 hours fever free, without fever reducing medicine Feels well enough to fully participate in school
